Case Studies

Promoting Utilization of Neighborhood Medical Clinics

The goal of the plan was to raise awareness of the clinics, increase patient volume, and help patients establish primary medical homes for regular care.

Addressing Disparities in Health Outcomes

LPHI addresses health disparities by helping communities develop solutions based on the needs of their unique populations.

Improving Technology at Community Clinics

A team from LPHI provided critical technical assistance to several primary care community clinics in the New Orleans area. This is part of a larger effort to shift providers to a system of interoperable electronic medical records.

Partnering to Measure Post-storm Population

After the hurricanes of 2005, LPHI used partnerships and mapping technology to help meet the need for timely, accurate population estimates.

Leveraging Funds: A Model for Public-Private Collaboration

The Primary Care Access and Stabilization Grant is strengthening access to quality health care in Greater New Orleans.

Building Biking Infrastructure in New Orleans

As a partner of The Regional Planning Commission’s Greater New Orleans Pedestrian & Bicycle Program (RPC), LPHI coordinated the official opening of the Wisner Boulevard pedestrian and bicycle trail in New Orleans.

A Comprehensive and Coordinated Statewide Approach

The Louisiana Campaign for Tobacco-Free Living (TFL) serves as a model for how community partnerships can reach across Louisiana.

Promoting Healthy and Active Lifestyles

Steps to a Healthier New Orleans worked to reduce the burden of chronic illnesses by promoting healthier lifestyles.

Collaborative Leadership to Strengthen the Behavioral Health Community

The Behavioral Health Action Network's ongoing collaborative effort aims to restore behavioral health services post-Katrina.
