New Orleans Charitable Health Fund
Program Overview
The Louisiana Public Health Institute (LPHI) on behalf of the New Orleans Charitable Health Fund (NOCHF) is pleased to announce the availability of up to $5.5 million in total funding to support integrated behavioral health, primary health care and referrals to social services efforts over a three year grant period (July 2012 – June 2015).
The NOCHF goals are to:
- Increase access to care. Provide easy and readily available access to integrated behavioral health, primary health care, and referrals to social services.
- Improve population health. Implement interventions and systems changes to improve health outcomes for the overall community.
- Promote sustainable systems change. Create lasting clinical and community interventions and systems change for the purposes of integrated behavioral health, primary health care, and referrals to social services.
