In 2015, LPHI launched the Community-Centered Health Homes (CCHH) Demonstration Project, under the Gulf Region Health Outreach Program (GRHOP), with five community health centers spanning four states in the Gulf Coast. The CCHH model, developed by Prevention Institute, aims to build the capacity of healthcare organizations to become active participants in improving community conditions that impact health, such as education and healthy food access. This is the first coordinated demonstration of the model in the nation. LPHI is excited to share a new report detailing the findings from the program.
