Regional Care Collaborative

Clinical Transformation’s Regional Care Collaborative provides the space for healthcare delivery professionals to share information, evaluate different ideas and network in an effort to benefit from one another’s resources and skills.

The Primary Care Capacity Project’s Regional Care Collaborative is an annual, two-day forum bringing together approximately 150 participants including physicians, social workers, program managers, and administrators from clinics and partners across the Gulf Coast states to learn from and collaborate with their peers and national experts.

At a recent event, the team offered two conference tracks of Sustaining Patient-Centered Medical Home and Improving Population Health in order to appeal to a diverse community health center audience. Recognizing that no two health centers are the same, topics ranged from Revenue Cycle Improvement to Community Assessments. Ample time was also given for networking and sharing among colleagues.

Post-event feedback from participants showed that a large majority found both the content engaging and the time well spent.
