General
Before opting into the ‘Food Is Medicine’ study, you will have been provided the opportunity to review information about participating in the study. One condition to participating in the study was to share your name and phone number so that study organizers could contact you by phone or email. Automated messages may be sent using an automatic telephone dialing system to the mobile telephone number you provided when signing up.

Message frequency varies, and additional mobile messages may be sent periodically based on your interaction with Food Is Medicine study materials. LPHI reserves the right to alter the frequency of messages sent at any time to increase or decrease the total number of sent messages. LPHI also reserves the right to change the short code or phone number where messages are sent.

Message and data rates may apply. If you have any questions about your text plan or data plan, it is best to contact your wireless provider. Your wireless provider is not liable for delayed or undelivered messages. 

Carriers 
Carriers are not liable for delayed or undelivered messages. 

Cancellation
You can cancel receipt of SMS messages any time by texting “STOP”. After you send the SMS message “STOP”, we will send you a message to confirm that you have been unsubscribed and no more messages will be sent. This does not remove participation from the study. To cancel participation in the study, please email: FIM@lphi.org. If you would like to receive SMS messages from LPHI again, please email: FIM@lphi.org and LPHI will start sending messages to you again.

Transfer of Number
You agree that before changing your mobile number or transferring your mobile number to another individual, you will either reply “STOP” from the original number or notify us of your old number at FIM@lphi.org. The duty to inform us based on the above events is a condition of using this service to receive messages.

Messaging Terms Changes
We reserve the right to change or terminate our messaging program at any time. We also reserve the right to update these Messaging Terms at any time. Such changes will be effective immediately upon posting. Your continued enrollment following such changes shall constitute your acceptance of such changes.

LPHI Food Is Medicine Privacy Policy
In all research, there is a possible risk of the loss of confidentiality, but we will do our best to keep your information secure. 

We will keep your personal information secure in the data system. You will be assigned a unique code that will be the link between you and your study information. This process keeps your health information and study results separate from your other personal information. Your personal information will be kept securely and will not be made available to anyone except for reasons described in this consent form.

Your privacy is very important to us. Here are just a few of the steps we will take to protect it:

1. We will have your name so we can reach you in the event you forget to complete a survey. We will share components of your personally identifiable information such as phone number and/or email address with the vendor of the intervention group that you are assigned to – either Instacart or Rouses. The vendors need this contact information to make sure you get your shopping funds each month. 

2. We will store study information on computers with controls to protect data confidentiality. We will limit and keep track of who sees this information.

3. Researchers who analyze the data to answer study research questions will not know who you are. The information that they get will be deidentified and not contain your name or other personally identifiable information. You will only be identified in the analytic dataset by a ParticipantID.
