LPHI Joins Pathways 2 Population Health Collaborative

It is with great excitement that the Louisiana Public Health Institute (LPHI) announces our participation as a Pioneer Sponsor in the Pathways 2 Population Health (P2PH) collaborative.

Pathways 2 Population Health is a collaboration among five organizations: American Hospital Association/Health Research and Educational Trust, Institute for Healthcare Improvement, Network for Regional Healthcare Improvement, Public Health Institute, and Stakeholder Health. This partnership will allow the leading organizations to leverage shared assets and unique strengths to help health care organizations accelerate their individual and collective population health improvement efforts.

This collaboration is facilitated by 100 Million Healthier Lives, a movement of change agents working to transform the way we think and act to create health, well-being, and equity.

LPHI is honored to join this effort by serving as a Pioneer Sponsor of the P2PH movement, leading the way by spreading the word and engaging others in this movement.
