LPHI was awarded $13.5 million over three years by the HHS Office of the National Coordinator for Health Information Technology to enhance primary care coordination through innovative health information exchange between private hospitals and community health clinics. The Crescent City Beacon Community (CCBC), was one of only 17 Beacon communities selected across the nation.

  • Initiated the region's first real-time automatic notification system from emergency department to patient-centered medical homes to facilitate transitions of care
  • Implemented a successful clinical transformation practice coaching program in 16 clinics leading to improvements in process measures
  • Operationalized a fully functional, clinical data exchange through the Greater New Orleans Health Information Exchange (GNOHIE).
  • Houses more than 900,000 patient records, approximately half of whom have had a hospital, clinic encounter and/or visited an outpatient clinic in the past two years.
