Louisiana Reentry Initiative

In 2016, a four agency collaboration (LPHI, Women with a Vision, CrescentCare, and the Capitol Area Reentry Program) completed a three year retention in care project, dubbed the Louisiana Reentry Initiative. Funded by AIDS United and the Corporation for National and Community Service, the initiative worked to retain formerly incarcerated individuals living with HIV infection in New Orleans and Baton Rouge in HIV care using three client-centered intervention models (health navigation, pre/post release case management, and community health worker/outreach).

Of the 144 PLWH receiving one of three client-centered interventions, 83% were linked to HIV care and 60% were retained in HIV care over a 12-month period.
