Community-Centered Health Homes Demonstration Project’s Preliminary Findings & Recommendations

In 2015, LPHI launched the Community-Centered Health Homes (CCHH) Demonstration Project, under the larger Gulf Region Health Outreach Program (GRHOP), with five community health centers spanning four states in the Gulf Coast. CCHH, a model developed by the Prevention Institute, aims to build the capacity of healthcare organizations to become active participants in improving community conditions that impact health, such as education and healthy food access. This is the first coordinated demonstration of the model in the nation. LPHI is excited to share a new report detailing early findings and recommendations for implementing and advancing CCHH.

Note: GRHOP was funded through the Deepwater Horizon Medical Benefits Class Action Settlement which was approved by the U.S. District Court in New Orleans on January 11, 2013.
